ALIGNMENT
Clear alignment with the agency mission and United Way Vision Areas

PROBLEM STATEMENT
The ability to identify the community problem the program addresses, and the extent of need for the service this program provides to the targeted population

STAFFING
Qualified staff/volunteers and appropriate supervisory structure

SPACE
Adequate materials and physical space

METHODOLOGY
Clear rationale for chosen methodology and clearly defined activities
Culture Evidence of connection to the target population and culturally competent practices

COLLABORATION
Effective collaboration with other agencies/programs where appropriate

OUTPUTS
Reasonable outputs given the program’s activities and budget

OUTCOMES
Clearly defined outcomes, including at least one United Way identified outcome, that focus on meaningful, reasonable, and attainable change for the client population

INDICATORS
An outcome measurement system, including indicators and data collection tools and procedures, that produces valid and reliable data

HISTORY
History of outcome data that indicates satisfactory progress for consumers

CONTINUAL IMPROVEMENT
The use of outcome information and participant feedback to improve service delivery

FUNDING NEED
The importance of United Way funding and detail usage for proposed funds

BUDGET
Realistic, clear, and consistent program budgets that adequately allow for the recruitment/retention of qualified staff.

OTHER REQUIREMENTS as may be noted in the Program Assessment
